The MAMA-MIA Challenge: Advancing Generalizability and Fairness in Breast MRI Tumor Segmentation and Treatment Response Prediction

Breast cancer is the most frequently diagnosed malignancy among women worldwide and a leading cause of cancer-related mortality. Dynamic contrast-enhanced magnetic resonance imaging plays a central role in tumor characterization and treatment monitoring, particularly in patients receiving neoadjuvant chemotherapy. However, existing artificial intelligence models for breast magnetic resonance imaging are often developed using single-center data and evaluated using aggregate performance metrics, limiting their generalizability and obscuring potential performance disparities across demographic subgroups. The MAMA-MIA Challenge was designed to address these limitations by introducing a large-scale benchmark that jointly evaluates primary tumor segmentation and prediction of pathologic complete response using pre-treatment magnetic resonance imaging only. The training cohort comprised 1,506 patients from multiple institutions in the United States, while evaluation was conducted on an external test set of 574 patients from three independent European centers to assess cross-continental and cross-institutional generalization. A unified scoring framework combined predictive performance with subgroup consistency across age, menopausal status, and breast density. Twenty-six international teams participated in the final evaluation phase. Results demonstrate substantial performance variability under external testing and reveal trade-offs between overall accuracy and subgroup fairness. The challenge provides standardized datasets, evaluation protocols, and public resources to promote the development of robust and equitable artificial intelligence systems for breast cancer imaging.

Medical Imaging AI Competitions Lack Fairness

Benchmarking competitions are central to the development of artificial intelligence (AI) in medical imaging, defining performance standards and shaping methodological progress. However, it remains unclear whether these benchmarks provide data that are sufficiently representative, accessible, and reusable to support clinically meaningful AI. In this work, we assess fairness along two complementary dimensions: (1) whether challenge datasets are representative of real-world clinical diversity, and (2) whether they are accessible and legally reusable in line with the FAIR principles. To address this question, we conducted a large-scale systematic study of 241 biomedical image analysis challenges comprising 458 tasks across 19 imaging modalities. Our findings show substantial biases in dataset composition, including geographic location, modality-, and problem type-related biases, indicating that current benchmarks do not adequately reflect real-world clinical diversity. Despite their widespread influence, challenge datasets were frequently constrained by restrictive or ambiguous access conditions, inconsistent or non-compliant licensing practices, and incomplete documentation, limiting reproducibility and long-term reuse. Together, these shortcomings expose foundational fairness limitations in our benchmarking ecosystem and highlight a disconnect between leaderboard success and clinical relevance.

Navigating the landscape of multimodal AI in medicine: a scoping review on technical challenges and clinical applications

Recent technological advances in healthcare have led to unprecedented growth in patient data quantity and diversity. While artificial intelligence (AI) models have shown promising results in analyzing individual data modalities, there is increasing recognition that models integrating multiple complementary data sources, so-called multimodal AI, could enhance clinical decision-making. This scoping review examines the landscape of deep learning-based multimodal AI applications across the medical domain, analyzing 432 papers published between 2018 and 2024. We provide an extensive overview of multimodal AI development across different medical disciplines, examining various architectural approaches, fusion strategies, and common application areas. Our analysis reveals that multimodal AI models consistently outperform their unimodal counterparts, with an average improvement of 6.2 percentage points in AUC. However, several challenges persist, including cross-departmental coordination, heterogeneous data characteristics, and incomplete datasets. We critically assess the technical and practical challenges in developing multimodal AI systems and discuss potential strategies for their clinical implementation, including a brief overview of commercially available multimodal AI models for clinical decision-making. Additionally, we identify key factors driving multimodal AI development and propose recommendations to accelerate the field's maturation. This review provides researchers and clinicians with a thorough understanding of the current state, challenges, and future directions of multimodal AI in medicine.
